As a healthcare provider, we are committed to protecting your personal information and ensuring you understand how information collected by your practice is used.
We collect personal information that is necessary to provide appropriate care and treatment. Your medical record is confidential. We maintain the privacy and security of your health information and insure it is accessed only by authorized staff.

Billing Policy
We are a mixed billing practice, which means consultations may incur an out-of-pocked gap fee.
Specialized consultations, including chronic pain, musculoskeletal and sports medicine appointments, may attract additional fees.
Procedures may also incur additional fees, depending on the type of procedure required.
We will advise you of any additional fees before your procedure.

Transfer of Notes
To transfer your medical records from a previous clinic, please contact reception. We can provide a form to help request your records. A health summary is usually provided at no charge; however, some clinics may charge a fee to transfer full records.
If you would like your records transferred from our clinic to another clinic, please contact your new clinic for a request form and ask them to send it to us. We will provide a health summary free of charge. A fee applies for a full record transfer. For more information, please speak with our reception team.
